Download and complete the order form, then forward your cheque or money order made payable to Children by Choice and mail to PO Box 2005, Windsor QLD 4030.  Please allow 3-5 days for delivery. Alternatively you could phone us on 3357 9933 and arrange a time between Monday - Friday 9am - 5pm to drop by the office at 237 Lutwyche Road, Windsor and pick up your order.
